Darwin Coronial Inquest findings

Airservices Australia wishes to extend our sincere condolences to the families of the deceased Lena Yali, Greg McNamara and Kevin Taylor.

We would also like to extend our sympathies to the survivor of the accident, Philip Harris and his family as well as our own staff, Jack Norris, Ashley Williams and Richard McCagh and their respective families.

Airservices acknowledges Coroner Cavanagh’s findings and recommendations and has already been actively working to address the key areas outlined in the Coroner’s Report.

Specifically we have taken immediate action to improve our driving policies when operating on public roads and driver training curriculum to include customised driver training of Airservices Aviation Rescue and Fire Fighting vehicles under emergency response conditions.

We are investigating improvements to vehicle design, including blind spots arising from the A-Pillar, side mirror design and position, vehicle markings and emergency lights and siren.

We are also further expanding our public awareness program to assist the community identify our vehicles and understand the critical safety role Airservices aviation rescue fire fighters undertake on behalf of the community.
